water leak on GE refrigerator supply

the existing 1/4"OD poly tubing feeding the refrigerator split. it appears the entire length was part of a kit installed for the previous homeowners refrig (similar to the GE smartconnect that I could use for my refrig) but I don't know more details than that. I bought standard 1/4"OD poly tubing and two 1/4" OD union fittings with inserts from a local bix box store to splice in. when returning water pressure to the line, the old tubing end leaks and eventually pops out of the insert under pressure. there are no issues with the new tubing side of each connection. before I run all new tubing, is there something I am missing?

May be something with the old tubing...but..

Are you using the brass inserts and ferrulles with the coupling? Copper lines don't need the inserts, poly lines do.

btw, I'd run all new lines or use stainless flex hose if it would reach. The splicing just adds 4 more possible failure points. And if the old line split once, who's to say it won't do it again when yer away for the weekend?
