How do you make a transition threshold from carpet to wooden floors?

It depends.
Did you cover the wood floor with carpet and now you want to finish it off at the doorway. In which case you can use a crimp metal, Z-bar and tackstrip, or turning and tacking. I have seen people actually use ¼ round and mill, sand and custom fit it. Didn't look bad, but have no idea how long it will last.
If it is over concrete and you have engineered wood glued down and installed carpet up to it, then a reducer or "T" wood molding to match the wood floor would look best.
