Any suggestions on how to paint behind the toilet?

Short of removing the tank and/or the toilet, you can remove the lid and edge out the best you can around and behind the toilet. You can tape of the edges of the tank to keep the paint off. I have lived in many houses where, when the toilet was removed, I discovered different paint color and sometimes wallpaper.

They also make a roller especially for this.

I always cut in with my trim brush!

actually the loo brush works quite well.:D
