I want to install tiles in my foyer, I ripped up the old lino and found floor construction to be, 3/8" plywood over 3/4" T & G 1 x 6. Will one more layer of 3/8" plywood over top be sufficient to install 12" tiles on? or do I have to use cement board?

Don't add more plywood, what you have is sufficient. Thinset and screw 1/2" cement backerboard to the plywood and then tile.
