How difficult is it to pull up 12 x 12 ceramic tile over cement in a kitchen? I'm weighing whether to pull it up or go right over the top of it.

It's not easy. Jack hammer and lots of blisters, not to mention everything in your house is going to have dust on it. I have had to use an angle grinder to get thinset off the concrete, once the tile is up. It takes time and lots of tools to do it properly and not ½ A$$ed. That's why I charge double for taking up old C-tile and thinset, then I do to install it.
