If I am going to use Joist holders to attach joists to the ledger, how do the joists attach to the girder. On top as the web page shows, or by joist holders? Also, what is a rule of thumb for spacing of piers? say for a 12x28 foot deck?

Joists on top of girder, and toe nailed in place.
