I have hardwood floors that were refinished three years ago. Now the finish has worn through in some of the high traffic areas. The wood, even in these areas, is still in very good shape. The floors were originally sealed and finished with water-based polyurethane.

What should I do to repair these worn areas, without refinishing the entire floor??

This is a quick fix for a fairly small area and on a high gloss floor.
Use the product called "Future" by Johnson wax. It is a clear product that dries to a high gloss. It is like a liquid plastic. It is actually acrylic. Put it on in thin coats. It dries very quickly and can be purchased in any grocery store. I dont't think this product would be recommended by wood people but I used it for years on a high traffic room with 50 yr old wood floors and it was wonderful. I recoated my floor with "Furture" about once a year. "Future" can be removed with ammonia, but that is a lousy job. I was actually putting it on an extremely scratched finish not on bare wood.
