How much separation (clearance) is required when a double- walled three inch vent pipe from a gas hot water heater passes through an inside wall? Must a special metal through-wall mounting be used or are those mainly for single-walled vents?

You'll have to check your state and local codes for that. Here it's min. 1" from anything flamable.
