I have very good plaster & lath in an older two story home with metal siding, and wondering what the best way to insulate it would be?

Though you didn't say it, I assume you don't want to disturb the lathe and plaster. (Patching that could be a real pain!) Contact an insulation contractor and get a quote about blowing cellulose into the walls from the outside. You'll have to remove the siding at the top of the walls, drill holes and blow cellulose into the cavities between the studs, plug the holes, and rehang the siding. Done right, nobody will ever know you were there, EXCEPT for the person paying the utility bills!!
