I removed parque flooring to lay 3/4" hardwoods. There are several spots where the subfloor (3/4" plywood) peeled away with the parque. Do I apply a floor leveler to the recessed spots and if so what type? Also, do I need to worry about the residual glue left from the parque flooring?

Chris, you want a subfloor than is flat, smooth, & sound. Unless the remainng glue is thin and smooth, it needs to come up. Don't you just love these rocket scientists who glue directly to the subfloor? Anyway, if the floor has alot of damage from the pull up you may have to cut the damaged sections out and replace them. It's not a good idea to nail down hardwood to floor leveler, as it won't hold nails very well and it could break up.
