Due to multiple allergies I am replacing berber carpets with bamboo flooring. I am pretty sure there is 5/8" plywood as the subfloor (home is 10 yrs old and located in Massachusetts). I am looking to eliminate as much as possible any chemical exposure. Any suggestions are welcome.

Welcome to the forums! 5/8" subflooring is sub minimal at best. I would add at least 1/2" more at a 90 degrees to what is there presently, then add your flooring. You don't want the flooring to flex and give. Formaldehyde would be a minor element of today's construction methods, if at all. Removing the carpet will help with your allergies.

Look in the heating supply duct outlets for the ply's thickness. (Unless you have other than forced air). The glue is nasty but in different amounts: 93 Nevins Street (http://93nevins.com/healthhouse/construction_materials.html)
