The trim in my house is 17 years old, stained and polyurethaned. The stain still looks great, the polyurethane has worn down. Can I redo the polyurethane and if so, how?

Welcome to the forums!
All you need to do is scuff sand the poly and apply a fresh coat. Window sills may require more sanding but as long as you don't sand off any stain - 1-2 coats of poly will make them look new. Heavily used doors/casings may need to be cleaned prior to sanding.
