I've been googling and have not been able to find what I am looking for.

The hardware attaches to a "box of shelves" inside a kitchen cabinet and allows someone to open the cabinet door, pull straight out all the shelves at the same time and then lower the whole thing so a handicap person could reach the top shelf.

Any idea who makes these or who sells them?

Argh...I just saw this on Holmes on Homes...matter of fact the episode just aired again.
Can't find anything on his website. The ones I saw were wire shelving that was counterbalanced as you describe.

Anyway, any good independant kitchen design or handicap specialty store should be able to help.

Don't look for any help at the big box....it ain't there. Check here ADA Handicap Accessible Kitchens _ Wheelchair Accessible Kitchens _ Adjustable Shelving, Cabinets, Worktop and Appliance Lifts (http://www.barrierfree.org/ada_handicap_accessible_kitchens.htm) for ideas. One, you actually attach the cabinet to the bracket and it lowers in entirety. I saw the episode GG mentioned. They were cool, and would be better IMO, if you could find them.
