Can you confirm how I should wire the the humidstat to the furnace board?

I located the HUM terminal and that goes to the humidistat right? What goes from the furnace board to the humidifier to complete the circuit?

Do I need a transformer?

We are missing a few letters and numbers before the -072d.

I don't know what on your board with out a full model #.
