I am about to install cement backer board in a tub/shower and need to know if the cement board should start at the top of the lip which I used to screw in to the studs or below the lip?

First, you must install a moisture barrier BEHIND the cement board. This can be 6 mil poly, or 15 pound felt. The moisture barrier and the cement board should overlap down over the lip, down to about 1/4" above the tub deck. To make the cement board vertically plumb, you need to install lath spacer on top of the studs to fur that surface out.
