Is this ceramic tile or a vinyl type tile?

You could lay a floating type laminate over the tile, but you would have the added height to deal with. If the tile is not perfectly flat, your new floor will likely fail.

They rent floor tile remover machines at big box stores, and some electric tile chisels as well. If you do tear it up, it's going to take some elbow grease and time.

it is ceramic. how do you know if its level or not, also if ceramic is unlevel would't it be unlevel on rough slab also?

It doesn't have to be "level" it has to be flat. To measure this, just take a long level or a flat 2x4 and lay it flat on the floor. You should not see any light under the board.
