Any number of reasons come to mind as to what caused the problem. One is the grass that grows right up to the base of the tree without a mulch ring. Mowing equipment could have damaged the bark and allowed insects and/or disease to penetrate the tree. I do see what appear to be some round holes, but they could be from after the loss of bark.
