Yes, sewing may be the only way. Use either an upholstery or hand quilting thread in a color to match. Unfortunately, hand stitching is not strong enough to hold up to the stress of people sitting on a cushion often, so this would need to be redone again and again. Another thing you can try is buying some fusible tape (one sided) and try to position it on the back of the tear, matching the two sides as well as you can, and ironing it on. The tape may only work on cotton or cotton blends since it may not adhere to certain fabrics and also test the iron on a hidden area to make sure the fabric can take the heat, if you go this route.
