Sand finished ceiling is some 30 - 40 years old and has become loose in some areas. Can this stippled ceiling be repaired or must the finish all come off?

Welcome to the forums Tom
It can be repaired although it might prove difficult to make the repair completely disappear. You will need to scrape off any loose paint/texture, mud as needed, prime and paint. The sand texture additive gets mixed into the paint.
I haven't bought any sand texture in years but I assume it is still available at most paint stores.
