I've rebuilt the stairs in a narrow row home. I'd like to start the railing on the second step - in order to make the passage way between the 1st step and the wall feel wider. Is this a violation of any building codes? Will this create any other problems attaching the railings/banisters?

Yes, this is a violation of all building codes for stairs that I am aware of. You have to start the top of your stair rail on top of the floor above the first step. Tough luck.

Probably, but check local code. The rail is supposed to aid people all the way down the stairs, so terminating it early could be a safety problem.
