I replaced my sister-in-laws condenser fan and capacitor at the start of spring. Fan motor bad locked up and cap just in case. She called and that fan is not turning on all the time. Some times it will start and some times it won't. I had an extra cap. and replaced it this am fan took off. What could cause the cap. to go out so soon? Voltage checked correct.?

Sometimes you just get a bad one. If there was some sort of power surge such as a thunderstorm, capacitors will often go out.
