I'm installing 3/4" hardwood in 2nd story rooms. As I expected from the squeaking floors beneath the prior carpet installation, the subfloor installation is not only "ugly", but not even at the panel intersections.
I'm concerned that as little as 1/32" of an inch difference between two subfloor sections is going to be a problem.
Can I shim with cardboard or plane it down to relieve my concerns? Should I replace the entire subfloor?

Be sure to screw down the subfloor to get rid of any squeaks. after that it does not matter. The new floor is nailed tight to the subfloor. Just don't put a joint over a joint.

Yes, sand down the high seam joint edges.
