Just form one out of ductboard; tape and seal it thoroughly.

You can build one out of sheet metal. You will have to know the size and location of each duct beforehand. Insulate it well with ductboard so it does not sweat.

If I have this right you have 8" from the old plenum to the new unit. Get what is called S slips cut and slip up on old plenum all 4 sides. Cut sheet metal to slid up in slips 1"and down to new air handler. Do this on all 4 sides. Over lap corner1" also and you can put a piece of slip there. Put sheet metal screws in corner slips other slips and in the sheet metal to the blower handler. Or like said use duct board to make it.

Stop by a HVAC shop they would have the metal and slips or have scrap Duct board to use for this small job.
