On one of our bedroom walls, an outside wall, we found some mould I believe (black stuff and some paint off). It is right in the spot where the bed is tight up against the wall. How serious is that, it is the first time we found it.

If the mold is just on the surface then its not a big dill. but if it is in the wall its going to be more off a problem!

The mould is just on the surface but the big question is how to keep it from reoccuring? Is this common? Is it a sign of poor insulation?

I would recommend cleaning it thoroughly, then moving your bed away from the walll to allow ventilation. Leave it like that for a few weeks and see if the mold returns. If it returns, then there is a chance that it is inside your walls.
