I planned a remodel but when I went to purchase a tankless WH with a horizontal vent I found that I didn't have enough water pressure to use the unit. Can I vent my existing WH horizontally after a 12" rise?

What's your existing water heater?
The tankless units have induction motors which will allow horizonal venting of the unit.
If you existing water heater has a standard atmospheric type vent, it must be vented up.

wtkc, you said horizontal, but you can buy an adjustable vent elbow that can be adjusted to 45 Degrees (or any angle) or so instead of a 90 Degree ell. That way you are still going up while going sideways. Make sure you use double wall vent pipe. Hope this helps.

Check the National Fuel Gas Code book for specs on how much length of horizontal venting is allowed as a ratio of the total vertical rise. Straight horizontal vent out the wall definitely is not allowed except for units which are designed as direct vent, or power vent.
