Does a toilet flange need to be flush with the service of the floor? Like if my floor is tiled, should the flange be perfectly flush with the tile top or is it okay for it to be slightly higher?

It depends what you mean by slightly higher. If the flange is like an 1/8 inch or so you can work with it. Get yourself some cedar shingles [The Home Depot carries them], put the toilet down on the flange, use the shingles to level the bowl, tighten down the closet bolts, {be gentle] install the toilet until it feels snug. then you take a razor knife and cut the shingles even with the edge of the bowl, Caulk the seam. The higher the gap the worse it looks. Lots of luck.

You should have not problem with the flange as long as it is sitting on the tile and secured there. Buy the thin wax seal. They do sell toilet shims for leveling but unless you really need them, do not use them. as far as caulking or grouting, I suggest you leave the back side of the toilet open so you know early if you have a leak.
