I am going to make dining room drapes using a rather heavy-weight Waverly fabric. They will be lined, tied back and hang to the floor. I am going to put them on a straight rod at double the window's width. Should the rod pocket be sewn on separately or just made by making two rows of straight stitching though the fabric and lining? T How wide should the fabric be above the rod to make them look nice?

i hope these are just decorative side panels, because 2x fullness isn't enough if they'll be closed. 3 to 3 1/2 is what you'd want if they'll be closed.
the fabric above the rod pocket is called a header, and i like a nice 3" ruffle up there, myself. anything less will look skimpy, especially on full length draperies.
as for how to make lined draperies, i have no idea. but i'm sure you don't just use the face fabric for the front of the rod pocket and the lining for the back. and you don't just make a sort of pillow case, because you want the lining to be inset on the back, so that it won't be able to show. and you'll need a nice 4 or 6" hem on the bottom for weight so they'll hang nice. but i've never made anything lined, and i guess i've never studied one enough to remember what the heck they look like! so i really don't know! :o
i'd google "how to make lined draperies" and see what you can find. unless someone else knows..??
