Can someone tell me what guidelines to follow for position of garb bars in a bath and around a commode?

I have found that it works a lot better .If you get the person in there that will use them. Ask them what feels right for them to hold on to. For their height. Also if it is better for them to push up or like pull up. On a grab bar..
I found that I had to move some I put in. To better help my wife.

If you refer to the ICC/ANSI 117.1-1998, this will have the information you are looking for, that is if the building code you are using is the International Building Code. If this deals with Mix-Use buildings then you may need to refer to the Fair Housing Act Design Manual.
The area at the toilet for H/C is 5'-0" (ANSI 604.8.1.1) and the distance from the wall to the centerline of the toilet is 16" to 18" (ANSI 604.2).
The placement of the side mounted grab bar is 12"max from the back wall to the return of the grab bar the distance to the end of the grab bar is 54"min. So you end up with a 42"min. grab bar on the side and a 24"min grab bar behind the toilet (ANSI Fig. 604.5.1 & 604.5.2).
