It is about a 20 ft ceiling, and the fixture is
in the middle of the foyer, so I can't lean a
ladder against a wall and reach it, in order
to replace it.
What options do I have?

Tall A-frame ladder if you have the space to open it up. A stairway ladder if the space includes steps. A scaffold would work.
If you have a big rental yard in your neighborhood, you may also be able to rent a one-man electric lift platform. Slick if you can find one.
