What is the best way to repair a sidewalk that is heaving from tree roots
underneath. Any suggestions?

Dig down & cut the roots back on the tree side a foot away from the walk. This will stop the heaving, You might be able to lift out the section regrade and put the section back, or break it up and do it over, if it only raised a little I would leave it alone.
