Currently we have a 4" toilet flange connected to a 4" pipe that once around an elbow, changes to a 3" pipe. Can we change the 4" toilet flange and 4" pipe to a 3" toilet flange,keeping all our piping one size? Would there be any bad repercussions to doing this?

Totally unnecessary. Why go to the trouble? If it aint broke, don't fix it. Either 3" or 4" is O.K. in this case.

Sounds like someone has patched in a repair on your existing system.
Never in plumbing should piping go through a reduction inline of any system.
Produce an image of your situation. Or go to www.photobucket.com Is the piping PVC to Cast?
All closet flanges are 4 X 3 or 4 X 4.......with 4 always the first number.
