Anyone recognize this stove?

Greetings,
I bought this wood-burning insert 2nd-hand. It has the month and year stamped on a small plate in the front of it - 4 / 84. I have no idea what make or model this stove is; can anyone help?

I am hoping to buy a fan/blower for this stove, but I don't know where to look for it. Any advice would be greatly appreciated I have 2 pictures of the stove here:
