my brother has a 82 f150 it out of the blue started to screw up with an extreme rough idle it is worse when it is in drive, timing is not off, not overheating . it has new plugs wires rotor ect. i am confused?

ok not fuel filter or pump no vacuum leaks ????????? cylinders maybe?

might be obvious but check the oil? and/or oil pressure while running

the truck has taken a turn for the worse now it sounds like a train
Did you check the oil pressure as suggested?What engine is in the truck?Describe the noise better please.
