Framing and Sub-Flooring - floor in bathroom, floor in hallway, and floor in opposing bedroom

my wife and i are very disturbed by this.
the floor in the hall bathroom, in the hallway in front of the hall bath and the floor in the opposing bedroom all have very noticable "bubbles" in the flooring and the hall bath toilet is loose.
i took up a piece of vinyl tile in the hall and it showed particle or pressboard.
my question is (assuming the entire house is pressboard subflooring) how or waht is the best way to replace these "spots" or systematically replace the entire floor in every room in the house with first plywood then pressboard on top of that?
the house is at least 25 years old.
what would be the average cost (assuming the rooms are "standard size")
for replacing room by room?

In areas that are exposed to moisture I would suggest changing to plywood. Just match the size of the existing. Average room with the increase in lumber prices due to the hurricanes Plywood liquid nail and screws I would expect around $200.00 to 250.00 for a 12'x12' room depending on your area.
