do become dirty and wear out over time, but I would think that it would take a little longer than 18 months. i woudl take into a shop and have a professional cleaning done. The self cleaning heads also only work on the video drum (where the video heads are) and not the rest of the "tape path", such as audio heads, tape guides, and the erase heads. You could also try a wet-type cleaning tape, but I don't think it'll help much.
Good Luck!

He's got a hi-fi vcr, so the audio heads are combined into the video heads.
A shop is going to charge more than buying a new one.
VCR's are throwaways. You can buy a new hi-fi vcr for $50.
