Bricks, Masonry, Asphalt and Concrete - New patio over concrete

I have an existing concrete patio that is about 4 inches too low. I would like to raise the patio approximately 4 inches by either adding concrete pavers, brick and concrete, or a combination of the two. Can I place the pavers, concrete, and brick directly over the existing concrete? (The concrete is in good condition and has very few cracks). Do I need a bond breaker, sand, etc? If possible, how do I place the pavers, concrete, and brick?
