Would it be wise to glue and nail a plank of 5 ¼ " wide or would it be ok just to nail that size of wood to the plywood. Thanks.

What does the hardwood manufacturer recommend.
All manufacturers specs on what you can do are different. Have them fax you the information you request. That way you have it documented, if a problem ever arises.
