What width should the rough opening be framed for a 36" exterior door ?

I usually make my rough openings 37 3/4 wide.
This gives me 3/4" on each side for a 1x? to slide in there. I know others who make it 38", others go only 37".

How about the HEIGHT of the rough opening? I saw one place say just add 3/4 inch to the height of the door (so you can insert a 3/4 inch top jamb board) but another place said add 2 inches. Assuming the door itself is 6 ft 8 inches, how high would you make the rough opening?
