Pulled up carpet and found 5/8 particle board underlayment. Removed the particle board and without underlayment the exterior door thresholds will be too high in relation to the wood floor. Is it common to put plywood underlayment over the subfloor in this situation? Are there any other options?

If you have a solid subfloor now Min 3/4 in for installation you have two options. Put down the correct thickness of plywood to make up the height difference or the use of transiton pieces. I would recomend you add the layer of plywood. It makes for a cleaner look. If price is the issue then lay the flooring as it stands and live with the height difference, but with past experience it will bug you.
