I have an old towel bar that seems to be grouted onto the ceramic tile wall. How do I remove it without cracking the tile?

I wouldn't be too concerned about cracking the tile when you remove the towel bar. The grout that is holding it to the tile has already made reusing that tile impossible. If you can not find an exact match for the tile(s) involved, find a tile that contrasts with what you have. As far as removing the tile the towel bar is attatched to, use a small grout saw to remove the grout on all 4 sides the you should be able to carefully pry the tile off of the wall. Just use a thin set mortar to replace it.
