What steps/preparation are needed to install ceramic tile over existing ceramic tile? Does the existing tiles need to be sanded to remove the polished glaze or should they be primed? What will help the new tiles adhere to the old ones.

I have a similar question - I have old tile on the floor - probably in some kind of concrete mesh. There are no loose or cracked tiles - it's just ugly. It would be tough to take up and easy to go over.
I also have a marble threshold at the door that is flush with the existing floor. What do I do about that??
