What is the best method of repairing chair legs & rungs that are loose fitting? What causes them to come loose?

Pressure of use and natural wood expansion and contraction.
Take it apart, scrape and sand the old glue off.
Re-glue the chair, and tie string tightly around it to clamp it together while the glue sits up.
