Maybe not plans - but general specifications can be obtained from your local building inspector.

For instance, to meet BOCA code (which is used by a lot of communities) the slope can be no more than 1:12; 1' rise for every 12' of length, and be no less than 48" wide...

I have the specs. i am still looking for a plan for myself also.
your local united way will build one for you at no charge. also try calling your local churhes, lions club, etc. they will build it for you.
anyway, the specs are:
Slope: 12" out for each inch of drop minimum 20" max run without a flat break.
Width: 3 feet
Curbs: 2" on each side - full length
Handrails: 38" high
Turning Platform: 5' square
Surface: Non-stick surface
Landing: Level clean, clear surface wide enough to turn
Materials: Support columns 4: x 4" spaced 4' apart
Support Joist, 2" x 6"
Flooring: 2" x 4" spaced 1/4" apart for drainage
Depends on size of ramp.
