I am building a bar for my basement using glass block. I would like to pour a 4"x4" base of concrete to ensure the glass block will be installed on a level surface since the existing concrete slab is not quite level. The bar will be shaped in the form of a half circle and will be 8 feet wide. How would I build the form to pour the concrete to take the shape of a half circle?

I would think a strip of 1/4 inch plywood would be sufficient enough to hold 4" of concrete.Just make sure to stabilize it by blocking up the outside of the wood every foot or so.1/4 plywood should be bendable enough for your shape
