While I was putting in a ceiling fan, the drywall puched from the attic side out a bit on the ceiling, Do I push it back up and spackle, or dig it out and patch? It is about a 2"x 2" area.

Push it back in and spackle, you may want to use the mesh tape for this so that it will be solid when done.
